Technical stuff
Most computers have Adobe Acrobat Reader, which
you can use to view, print or download the blank forms. Or you can fill out
the interactive forms on-line, and print your completed form. Click on
the form you want and it will open in your Reader.
Get more information about downloading and using Adobe Acrobat Reader.
For the more complex forms, like the supplemental child support worksheet, we recommend
using the most recent version.
Want to save your completed form?
When you fill out our forms using Adobe Acrobat Reader, you can print them.
But you won't be able to save them. To save completed forms, we recommend using
Foxit ReaderorCutePDF.

INSTRUCTIONS FOR COMPLETING INTERACTIVE
FORMS

Point the cursor arrow to any box and click on it to check
or un-check that box.

Put the cursor on any line and click on it to fill out that
line. Some lines, like the signature and some docket number lines, cannot
be filled in on-line.

You can tab between lines using your tab and shift-tab keys.

You don't need to fill in every line on-line. Unfilled lines
will print out in blank and can be filled in later.

On some older forms with built in help, double-click on the help note
icon for help with preparing the different parts of the form. Click on
the little dash in the upper left hand corner of the help window to close
it. Many of the newer forms have help if you just hold the cursor over a line or
checkbox

On those older forms with built in help, when you print, be sure the
"annotations" box is not checked in the print window (if there
is one). Otherwise the little help icons will print as well.

Unfortunately, you cannot save partially completed forms using
Adobe Reader. To do that you will need third party software like
Foxit
(free), or CutePDF Form Filler (free trial version,
$29.95 full version). You can also save partially completed forms if
if you have the full version of
Adobe Acrobat (not just Adobe Reader).

The .pdf forms in this section are not interactive. They can
be printed and filled out by hand or with a typewriter:

can have serious consequences. If at all possible a person preparing a
complaint should get help from a domestic violence expert. Click

in six languages for comparison. They still must
be filed in English, however.

The forms in this section are "interactive" and can
be filled out "on line." The "self-calculating" forms
(child support affidavit and child support worksheet) require Adobe Acrobat
Reader version 4.0 for the self-calculating function to work. We want to make
these work well for our staff, volunteer attorneys and pro-se clients. Please
let us know ways we can improve how these interactive forms work. Send
us your ideas.
